1. How do I make AMCH my primary care-giver?

2. What kind of benefits will the members program provide?

3. Can I change caregivers at anytime for any reason?
Yes.

4. Do I have to notify my caregiver if I’m removing them as my sole caregiver?
Yes.

5. Does the registry send any notice to the caregiver?
No, it is up to the patient to do so.

6. Can I be my own care giver?
Yes, in addition to that you can also have a Primary Care Center (AMCH) as your caregiver effective July 1st 2010.

7. Can my physician be my care-giver?
No.

8. Where does all of AMCH medication come from?
All of our medication is locally grown from professional growers.

9. What constitutes me being a Legal Patient?

Both registry forms need to be complete by a physician and a notary, a CERTIFIED MAIL COPY of the reciept to CDPHE must be present with forms in order to receive service.
